Expungement Forms

Expungement Background Sheet and
Criminal History Waiver Form

Instructions to complete the expungement forms:

1. On the Expungement Background Sheet, 
   1. Fill the top portion which includes - Name, DOB, Race, Gender, SSN, Phone/Email
   2. Please enter SSN with an “and” instead of “-” so separate the clusters of numbers. For example, 123-45-6789 should be entered as 123 and 45 and 6789
   3. “Impact of Conviction” is optional and can be discussed with attorney on site.
2. On the Criminal History Waiver Form,
   1. Please complete and sign the form before submission. 
   2. Electronic signatures are acceptable
